We have no way of knowing just by looking at Derek what his situation is.

I happen to feel as LikeWhoa! does–I don’t see much difference in Derek/Davis except he’s a bit older and looks like he’s maybe lost muscle mass because he’s not working out as much. He doesn’t look sickly to me. I think he’s just a naturally thin person who wouldn’t have any bulk at all if he didn’t lift weights (just like Travis and Jared who are also unusually thin compared to the average adult male).

I did volunteer work at a Hospice facility for people with AIDS, and Derek’s sick looking, overly gaunt face, and alarming overall skin and bones look are scarey to me, not erotic.
