I believe some of them were MEN being sloppy/sketchy themselves with the labelling (remember when you pointed out they made Wesley/Michael scenes ambiguous because they seemingly tried to hide whether the scene was condom/BB in their promo pictures?) and some were WB being careless with it, but I don't know. My memory is a bit unreliable, but it feels like it happens for more times than the amount you mentioned.
